Description: I was northbound on Guerrero St with another cyclist 10-20 feet behind me. At 27th St a SUV waited to turn Left. In the other two lanes traffic was backed up, but clear of the box. The curb lane, my lane was clear. As I approached the intersection at about 15-20 mph the SUV pulled out in front of me. I hit the right rear side of the car with my bike, left shoulder and left side of my face. I then bounced off hitting the pavement with the back of my head. My helmet was crushed at the rear. I sustained a significant laceration to my left cheek, complete shoulder separation, a subarachnoid hematoma (bleed in my brain), and permanent amnesia of the preceding hour and following 22 hours. I responded to the ambulance personel with typical head trauma non cooperation. I did not recognize the paramedics nor the ER physician who I knew personally. I was hospitalized in the ICU for 2 days and 1 day in a regular ward. 2 operations and 14 months later I returned to work.
